As for having coffee or tea during your fast — you should be just fine. As a general rule of thumb, if you drink something with less than 50 calories, then your body will remain in the fasted state. So, your coffee with a splash of milk or cream is just fine. Tea should be no problem either.
For a regular intermittent fast in a quantity no more than two teaspoons, no. Also no added sugars.
But if the fast were for a medical reason like a blood test or surgery, even the coffee itself will ruin it, so yes, stick to water.
In a regular situation though, that will be totally fine.

If the fast is for a medical reason like a diabetic blood test, then you shouldn’t be having anything but water, and in hospital before an operation, you wouldn’t even get that.
If it’s for the purpose of an intermittent fasting diet, it’s fine. Keto is different and it would disrupt the effect of say a 16:8 regime. I don’t know so much about the rules of Keto, but I am on the 5:2 regime.
There is sugar in vanilla extract but the amount is so tiny, that enough to flavour a cup of black coffee, really isn’t significant even to bother counting.
If that is the purpose of your fast, don’t be too hard on yourself. We all need little treats and this is truly a tiny one.
Alcohol, like sugar, requires digestion and processing by the liver, and most extracts are alcohol-based. In theory, it’s not fasting if you have anything but water and minerals. But in the real world, most fasting is not made pointless by having a tiny number of calories. The thing to consider is that it’s not a polarity, a light switch. It’s not like you’re fasting or not doing it at all and there’s no point because you had coffee with vanilla. It’s more like, the overall effects of the fasting may be reduced. How much so is an unknown.

Yes. Having black coffee, itself, breaks a fast. If you are fasting for a medical test or procedure, black coffee is definitely something that you should not drink.. When fasting for any reason you should drink only plain, pure, clean water.
